Family Educational Rights and Privacy Act

Student records may only be released to the student or his/her designee as directed by the Family Educational Rights and Privacy Act of 1974 (FERPA). Personally identifiable information or records relating to a student will not be released to any individual, agency, or organization without the written consent of the student as described in FERPA regulations, except “Directory Information” which may be released unless the student specifically withholds permission to do so. To prevent directory information from being disclosed, students need to complete a “Request to Withhold Information Form”. Transcripts

Official and unofficial student transcripts are available upon written request by the student.

  • Requests may be made by mail, fax, or in person.
  • An official transcript cost $5 each and must be paid when the transcript request is submitted.
  • Unofficial transcripts can be printed online by the student when logged in to the online Student Portal.
  • Students cannot have an outstanding balance with the college.
  • Transcripts will be processed within 5-10 business days.
  • A picture ID must be provided in order to pick up the transcript.
  • Transcripts may be released to another individual only if the above listed conditions have been met and a FERPA Information Release Form accompanies the Request for Transcript Form. This form must be signed and dated by the student and include the name of the individual to whom the transcript is to be released. A picture ID will be required for pick up by individuals other than the student.

Certificates

Certificates are printed monthly and available for pick up in student services located at the Thanksgiving Point, Orem, and Spanish Fork campuses. Certificates are held for 6 months following the student’s completion date. Certificates can only be re-printed for students whose completion date is after July 1, 2007.

  • A duplicate certificate cost $5 each and must be paid when the certificate request is submitted.
  • Students cannot have an outstanding balance with the college.
  • Duplicate certificate requests will be processed within 5-10 business days.

Transferring Course Work

A student transferring from another UCAT regional campus or another campus of higher education must meet the admission requirements of the MATC program. Students who transfer and request recognition of outside coursework must provide official transcripts and any other records and/or reports for evaluation. Transfer of acquired competencies and skills will occur after review by and/or demonstration to MATC program instructor(s).

Record and Storage Disposal

All student academic records are retained, secured, and disposed of in accordance with local, state, and federal regulations. All student records information is maintained on MATC’s Student Information System at the main campus. MATC maintains records for each student which may include: previous education, attendance, progress, tuition and fees, demographics, training outcomes, competencies obtained, and certificates awarded.

Social Security Numbers

The College securely and confidentially maintains your social security number for routine uses such as facilitating document matching, verifying you identity, and expediting your enrollment. Disclosure of your Social Security Number is voluntary, but failure to provide it may result in delay or loss of tax credits, student
